A proposed step has one curved arrow: from a lone pair on hydroxide to the carbon bonded to Cl in CH₃CH₂–Cl. No other arrow is drawn. What is wrong?

Five bonds on one carbon. The arrow makes a new O–C bond, but that carbon already has four bonds. Unless a second arrow breaks the C–Cl bond and sends that pair to chlorine, the carbon would hold five bonds. Hydroxide does have lone pairs, and the tail belongs on them.
